How to Submit a Claim
Claims should be made as soon as possible to ensure prompt payment. Written notice of a claim must be submitted within ninety (90) days of the date the claim arises.Please consult your benefit booklet for coverage details.
All claims are to be sent to the Administrator and should clearly indicate the following:
- the name of the Plan, which is the Steelworkers Trusteed Benefit Plan";
- your name (the member), address and social insurance number;
- if your claim is for a dependent, the dependents first name, age and relationship to you.
You should consult you benefits booklet as to where to direct claims, but in general, all claims, EXCEPT prescription drugs, should be sent to:
