USW Local 113 Scholarship 2008

To:         All Retirees and Former Members of U.R.W. Local 113 Employed by Firestone as of July 1987

RE:        U.S.W. LOCAL 113 SCHOLARSHIP 2008

Please find enclosed application for the 2008 Local 113 Scholarships. As in past years, three (3) scholarships, each worth one thousand dollars ($1,000.00) will be awarded.  The details of application guidelines are enclosed with this letter.

In September of last year, the 2007 Local 113 Scholarships were awarded. Congratulations to Alyssa Moulaison, Peter Anthony Markes and Stephanie Amanda Tukonic, all of whom are grandchildren of former Local 113 members.

In order to continue to participate in this program, it is the responsibility of all former members to keep the United Steelworkers National Office advised of their current address.  The retiree or former member of Local 113 should notify their children, grandchildren and great-grandchildren that they are entitled to apply for this scholarship in the event of the death of the retiree or former member.  It then becomes the responsibility of their lineal descendants to keep the U.S.W. office informed of their addresses.

TO ALL U.S.W. LOCAL 113 SCHOLARSHIP APPLICANTS:

Please check to ensure that your scholarship application package contains the following items:

  • 1 copy of the Instruction Sheet: “How to Complete Application Forms” (3 pages)
  • 1 copy of the APPLICATION FORM (3 pages)
  • 1 copy of the Confidential Academic Evaluation Report (1 page)
  • 3 copies of the Letter of Reference    (3 pages)



If any of these items are missing, or if you have any questions regarding these documents or the scholarship in general, please call the National Office, Education Department, at: 416-487-1571

HOW TO COMPLETE THE FORMS

1.    APPLICATION FORM

  • Type or print neatly all the information on the form.
  • Do not leave any questions blank – this may jeopardize your chance in competition for this award.
  • Re: Question #9 These are the names and addresses of the 3 people you will be giving a Letter of Reference Form to.  Make sure that at least two of these people are teachers.
  • Re: Question #10 This is the name and title of the High School Official who will be completing your Academic Evaluation Report.   Please make sure you give the form to this official.
  • If you feel you have any SPECIAL information to include that is not asked for on the application form, please include a separate letter and attach it to the form, but limit yourself to one page.
  • RETURN YOUR COMPLETED APPLICATION FORM NO LATER THAN JUNE 1ST, 2008.
  • Re: Note #2 Your official high school transcript must be in our office by JULY 15TH, 2008.


2.     ACADEMIC EVALUATION REPORT

  • Give this report to the high school official as instructed above and on the form.


3.    LETTER OF REFERENCE FORM

  • Give a copy to each of your referees, two of whom must be teachers.
  • Ensure that all three of your references are aware of the deadline – they must complete the form and send it to our office by JULY 15TH, 2008.


IMPORTANT NOTE:

In past years, some applicants have not sent in all of the above documents and therefore, we could not judge them based on their incomplete files.  If you do not send all of these documents by the dates specified, you are automatically removed from consideration for the scholarship (unless you phone or write and specify a reason that certain documents will be late).


GUIDELINES FOR APPLYING FOR LOCAL 113 SCHOLARSHIP

  1. First preference is to former Local 113 members or their lineal descendants who are entering first year of university or community college in the fall of 2008.
  2. Must have an average of at least 75% in the final two years of high school.
  3. Applications must be submitted by June 1st, 2008.    
  4. Candidates are responsible for sending high school transcripts by July 15th,  2008.
  5. Evaluation will be based upon academic transcripts as well as confidential  report from the high school and letters of reference.



In the event that there are less than three (3) qualified applicants entering first year university or community college in the fall of 2008, the remaining scholarships shall be awarded in the following order:

a)    to applicants who are currently attending a university or community college and who were not awarded a scholarship, then to
b)    prior winners currently attending a university or community college, then to
c)    other applicants from District 6 who have not previously been awarded a U.S.W. scholarship.
